SCase Study

The patient was a 60-year-old man who was diagnosed with moderate COPD three years ago. He was admitted due to dyspnoea and a chronic cough. The patient has been smoking for the last 15 years and was undertaking a smoking cessation course suggested by his GP. The patient was prescribed a long-acting bronchodilator and a course of inhaled corticosteroids. The patient needed to be encouraged to participate actively in his self-management plan.

This patient sounds ideal for the purpose of the case study. Do have a little think about the principles of self-management support…. as this is much more about partnership working rather than encouraging someone to participate (which sounds a little like telling someone what to do). So do be careful to get the way of working correct.

Design a plan of self-management support to meet the person’s needs.

Step 3.
Write a 3000-word essay that critically analyses the self-management support plan you have designed for the person you have chosen.

Your essay should address the following five areas:

1. Discuss the key principles of self-management support, placing this in the context of local and national long-term conditions policy and initiatives.

2. Discuss the chosen person’s experience of the long-term and the associated physical, psychological and social impact.

3. Analyse the evidence and underlying principles for the self-management support plan you have designed. Consider the associated benefits and potential challenges.

4. Evaluate the quality of the self-management support plan that you have suggested and conclude with evidence-based recommendations about how people could be best supported to self-manage their long-term condition.

5. Ensure your work is fluent, accurate written in an appropriate academic style and supported by relevant reference material.
