Funding proposal for a community health development project
For this assignment, you will develop a funding proposal for the project / program of your choice.
From your experiences in the community health development practicum, identify a project for which you could develop a funding proposal. This project should have relevance / importance for Nurse Practitioners. Then, submit an application to the Enhancing Community Health Foundation (not a real organization) requesting funding for your project.

Request for Proposals

The Enhancing Community Health Foundation is a philanthropic foundation providing support for community-based projects that enhance the health of children, adults and older adults in communities across the country. The Foundation is pleased to issue a Request for Proposals for community development projects that will address health concerns of vulnerable populations of children, adults or older adults. The proposals must be developed according to the format outlined below. The proposal is to be a maximum of 3750 words, excluding title page, executive summary, references and appendices. Include a word count at the end of the proposal.

NOTE : You may choose to write the proposal in the same format of the agency where you may want to actually submit the proposal. In that case, you may choose NOT to use to APA format for the proposal; however, this MUST be discussed with the instructor before you submit the proposal for marking. If you don’t discuss this with your instructor, you will be graded according to APA guidelines.
Use the following format to present your proposal:

Proposal Content (80 points)
• Title Page: include the project title, project beginning and end dates, amount requested, and information about the principal applicant, such as position, organization, phone number and email address.

• Executive summary : on a separate page provide a brief (maximum 1 page) summary of the project’s problem statement, goals and objectives, target population, and expected outcomes. Do not include references. (5 marks)
• Description of the Project (45 marks):
o an overview of the project (what is the project and what is it intended to achieve) (10)
o a description of the problem with supporting documentation from literature and community assessment data (10)
o a description of the target population (10)
o project goals and objectives (7.5)
o project activities and timetable, including a GANTT chart (include as an appendix) (7.5)

• Project Evaluation : describe the evaluation plan that will be implemented with the project.
(10 marks)

• Dissemination of Project Results : describe how the results of the project will be reported to the Enhancing Community Health Foundation and disseminated to a broader, relevant community. (7.5 marks)

• Budget and budget justification: develop an in-kind, line-item budget identifying and providing justification for expenditures such as salaries, travel, facilities, equipment, supplies, advisory committee meeting costs, dissemination expenses, etc. (7.5 marks)

• Appendices : Any additional information, including maps, descriptions of populations, facilities, etc., to assist the reviewers to better understand the need and proposed project. Ensure that each is clearly presented and labelled appropriately as identified in your APA manual. Make sure that you link each appendix to the relevant section of your proposal.

Items that are may be attached as appendices include:

• When acronyms or terminology not in common use are in the proposal, a table or glossary listing may be useful to the reviewers.

• Timelines, including charts and elaboration of timelines not already included in the proposal. Along with the timeline, you may provide a work-plan of activities to be carried out.

• Budget justification.
• Continuation of the project once this funded project is completed.
• Community Advisory Committee, including Terms of Reference and Membership
• Information about the organization, including previous funding obtained, demonstrating the capabilities of the organization and the organization’s infrastructure demonstrating its ability to undertake the work proposed.
• Resume/curriculum vitae of key members of the team responsible for implementing and evaluating the proposed project.
• Letters of support for the proposal.
• Consent forms and Institutional Ethics Review if relevant.
• Any additional information, including maps, descriptions of populations, facilities, etc. to assist the reviewers to better understand the need and

proposed project.

• Signature sheet of the appropriate people that may include the head of the organization and project director.
Scholarly Writing and Critical Thinking (20 marks)

• The proposal should be a maximum of 3750 words, excluding title page, references and appendices.

• The proposal is structured according to APA format with a title page, introduction, body organized by appropriate headings, a conclusion, references and appendices if needed

• Spelling, grammar and punctuation are correct

• Ideas are clearly articulated and organized. Information is well organized such that logical links are evident within paragraphs, within sections and among sections of the proposal.

• Relevant scholarly evidence is used to support statements and is accurately cited and referenced
• The proposal shows evidence of critical thinking including analysis and synthesis, integration of theory and personal experiences and original thinking
