Assessment method An in-person assessment in the form of an individual 10 minute presentation via Microsoft Teams using PowerPoint and a submitted assessment script. 100% of total module mark.

Assessment title and outline

Critically discuss an experience from your practice placement, which involved the leadership and coordination of holistic care for a person with a complex health condition.

• You must base your work on a real life experience from your practice placement. • The leadership and co-ordination of care must include – prioritisation, negotiation and delegation – collaborative, multi-agency team working involving a nurse and other health and care professional(s) – effective person and team management (NMC Standards of Proficiency)

• The person with a complex health condition could be a patient, service user, parent or other family member.

1. State the aim of your presentation and how this will be achieved.

2. Identify and explore the holistic care needs of the person with a complex health condition.

3. Explain how those care needs are influenced by social, political and economic factors.

4. Discuss and evaluate the leadership style(s) displayed by the professionals, identifying strengths and weaknesses.

5. Consider actual and potential causes of concern, conflict or confrontation and how these were prevented or managed using the NMC Standards of Proficiency.

6. Reflect on the way in which collaborative, multi-agency team working influenced the provision of care and maintained safety and quality.
