Population Health Informatics

For your Final Project, you will create a proposal presentation for a hypothetical, integrated health care delivery system (IDN) of your choice based on one of the five pillars of health informatics suitable for a professional health care setting. You will research one of the pillars of health informatics and the needs of integrated health delivery systems that have both provider and insurance business lines. Through the development of a slide presentation and media-based executive summary, you will analyze the most substantive issues affecting a provider or payer in the health care industry. You will then formulate a comprehensive plan for a transformation agenda to transform the chosen organization. This plan will use informatics theories and methods that align with your chosen pillar of health informatics.

Choice based on one of the five pillars of health informatics suitable for a professional health care setting: Population Health Informatics
Your Capstone Final Project must include the following headings and components:

• Introduction and Problem Statement – The introduction provides the background and context for your proposed project. This is where you will identify the need for the project and describe how informatics theories, tools, and methods will transform the organization. Utilizing the Population Health Informatics pillar that you chose.
o Introduce your transformation plan to the integrated delivery system.
o Explain why the transformation is important.
o Summarize the need for transformation using health informatics methods.

• Description of Theoretical Basis: conducted a peer-reviewed literature review on the published research on the chosen pillar that included a minimum of ten peer-reviewed articles. Based on the work you did in that review complete the following in this section:
o Summarize the literature basis for choosing your approach.

• Scope of the Proposed Solution: Based on the knowledge and skill sets you developed in your previous course, complete the following in this section:
o Identify whether your proposed transformation agenda for the integrated delivery system will be executed in a project or program.
o Describe the scope of the proposed project or program based on the informatics data sets, technologies, and methods chosen.
• Evaluation of Data Sets:
o Provide specific examples of the internal and external data assets you plan to use.
o Assess the data mining strategies you plan to use to acquire data from these data sets.
o Justify why these assets are imperative to your transformation agenda for the organization.
o Identify the methods you plan to use to analyze these data assets and your plan for transforming these data into meaningful information using data mining strategies.

• Evaluation of Technology Platforms – Complete the following in this section:
o Propose the specific health information technology platforms that you would incorporate as part of your proposed transformation agenda.
o Explain health information technology applications as well as technology infrastructure needed to execute your plan.

• Environmental Analysis: Stakeholders and Culture:
o Explain how stakeholders are affected by your proposed informatics technologies and methods.
o Evaluate the cultural elements that affect your internal executive stakeholders and their expectations of using the informatics solutions.
o Evaluate the cultural elements that affect your consumer and patient stakeholders’ use of the health informatics platforms that you propose.
• Statement of Implementation Approach:

o Identify an implementation approach that achieves your idea for innovation and transformation the IDN that you choose.
o Formulate the implementation approach that best aligns with your stakeholders’ expected outcomes.

• Plan for Financial Sustainability:
o Consider all costs associated with the use of both current and new technology platforms and/or data sets that are included in your proposed transformational plan.
o Develop the capital and operating costs of your implementation approach over a five-year period.
o Incorporate external forces information by discussing at least three cultural, social, legal, or health care policy forces that affect your financial pro forma.
• Plan for Identification and Mitigation of Risk:
o Formulate a plan to document risks and high-level risk mitigation strategies related to your proposed transformation proposal
o Analyze risks that are technical, operational, brand related, and financial in nature.
o Propose a mitigation strategy for at least two specific risks associated with cultural, social, legal, or health care policy factors that affect your choice of an informatics solution.

• Plan for Program Evaluation:
o Develop a recommended plan to evaluate your proposed transformation activities.
o Evaluate relevant qualitative and quantitative measures that align closely to the outcomes that your stakeholders expect to gain from the strategic use of data assets and/or informatics platforms. (Remember to include the costs of these evaluation activities in your Plan for Financial Sustainability.)

• Communication Plan:
o Explain your target audiences, your messaging, and your modalities of communication for each of these audiences.

• Executive Summary:
o Summarize your project in one to two slides that are appropriate for a senior executive audience.
o Discuss your data driven approach and expected outcomes using graphics and summary data.
The executive summary slides must encapsulate your unique approach to this transformation agenda for your critical executive stakeholder audience.
