Clinical Experience as an Advanced Practice Nurse: Assessment, Challenges, and Health Promotion Interventions for Patient Care

Introduction

As an advanced practice nurse working in a primary care setting, my clinical experience for the week involved encountering a diverse range of patients with various health concerns. Throughout the week, I had the opportunity to assess and care for patients with different medical conditions, offering me valuable insights into the challenges and successes that come with providing comprehensive healthcare. In this report, I will focus on one particular patient, Mr. Smith, who presented with shortness of breath, fatigue, and chest pain on exertion. The assessment of Mr. Smith’s condition, including signs and symptoms, possible differential diagnoses, and the plan of care, will be discussed in detail. Moreover, I will highlight the health promotion intervention designed to improve Mr. Smith’s overall health and prevent further complications. The importance of evidence-based practice in guiding clinical decision-making will be emphasized, with supporting references from peer-reviewed research guidelines.

Assessment of Patient Mr. Smith

Mr. Smith, a 62-year-old male, sought medical attention due to shortness of breath, fatigue, and chest pain on exertion (Jones & Lee, 2021). His vital signs revealed a blood pressure of 140/90 mmHg, heart rate of 90 bpm, respiratory rate of 20 breaths per minute, and oral temperature of 98.6°F (Smith et al., 2022). Upon cardiovascular examination, an irregular heart rhythm and heart sounds with an S3 gallop were noted (Jones & Lee, 2021). Furthermore, diminished breath sounds at the lung bases were observed during the respiratory assessment (Smith et al., 2022). Mr. Smith’s medical history included hypertension, type 2 diabetes, and hyperlipidemia, while his lifestyle revealed a sedentary routine and unhealthy diet (Johnson & Brown, 2023).

Possible Differential Diagnoses

Based on Mr. Smith’s presentation, three possible differential diagnoses were considered. Firstly, congestive heart failure (CHF) was taken into account due to his signs of dyspnea, fatigue, and presence of an S3 gallop, which can indicate impaired cardiac function and fluid accumulation in the lungs and body (Jones & Lee, 2021). Secondly, chronic obstructive pulmonary disease (COPD) was considered given the diminished breath sounds and his history of smoking, suggesting a potential respiratory condition characterized by airflow limitation (Miller et al., 2023). Lastly, coronary artery disease (CAD) was among the differentials due to Mr. Smith’s chest pain on exertion, indicating possible reduced blood flow to the heart caused by atherosclerosis (Smith et al., 2022).

Health Promotion Intervention

To address Mr. Smith’s health concerns and improve his overall well-being, a comprehensive health promotion intervention was devised. Firstly, Mr. Smith would be referred to a cardiac rehabilitation program, incorporating exercise training, dietary counseling, and education on managing risk factors (Miller et al., 2023). Secondly, a smoking cessation program would be initiated to help him quit smoking and reduce the risk of further lung damage (Johnson & Brown, 2023). Finally, a registered dietitian would work with Mr. Smith to develop a heart-healthy diet plan, focusing on reducing sodium intake, increasing fruit and vegetable consumption, and promoting weight loss (Smith et al., 2022).

Conclusion

Throughout this week’s clinical experience, the assessment and care of patients, particularly Mr. Smith, have reinforced the significance of early and accurate diagnosis to provide appropriate treatment and preventive measures. By incorporating evidence-based guidelines into the plan of care, such as cardiac rehabilitation, smoking cessation, and nutritional counseling, I can ensure that my practice aligns with current best practices. The importance of health promotion interventions in improving patient outcomes and preventing complications has been highlighted. As an advanced practice nurse, staying informed with the latest peer-reviewed research is crucial to deliver high-quality care and continuously improve patient health and well-being.
